ACCOMPLISHMENTS/ACTIVITIES

You may prepare your responses for Academic Honors, Work Experience, Extra-curricular Activities, Community Service, College/University Choices, and the Essays using a word processor, where you can utilize spell checking and grammar checking tools, and then cut and paste them into the designated boxes for each answer.

Please read and follow the directions carefully. When asked to LIST ITEMS BY YEAR, do not provide a narrative, just a listing of ALL specific items that are applicable for each of your high school years.

List All Academic Honors Achieved, Summarized By Year (e.g., Awards, Honor Roll, National Honor Society, etc.):

List All Work Experience (Include Part Time, Summer Employment, And Number Of Hours Worked Per Week), Summarized By Year And Indicate Whether It Was Summer Employment Or During The School Year:

List All Extracurricular Activities Within School (e.g., Clubs, Sports Teams, Student Government, School Newspaper, Leadership Positions, etc.), Summarized By Year:

List All Community Service And Activities Outside Of School (e.g., Church Groups, Social Clubs, Tutoring Programs, Special Hobbies, etc.), Summarized By Year:

List All Colleges/Universities To Which You Have Actually Applied And Note Your First Choice:

(NOTE: Since we provide our partner colleges with a list of all applicants who have applied to a given college, it is important that you only include those colleges that you have actually sent an application to or plan to apply to.

Our partner colleges also require that you provide your FAFSA Form (some also require the CSS Profile Form) so they can determine your financial need.)

ESSAYS

(NOTE: Comprehensive, well developed, and concise essays are better than longer, run-on, and repetitive essays.

When providing answers to the ESSAYS, please include all the information requested. If the question says to include two people you would have accompany you, be sure to include where you want to go, name the two people and be specific about who they are and why you included them.)

Essay #1: Write a letter to your first-year roommate at college. Tell him/her what it will be like to live with you (be sure to highlight what you believe are your best and worst attributes), what you are most looking forward to about college, your planned major, whether you can speak any languages other than English, and why you believe you were awarded the Deeley Foundation scholarship.

Essay #2: If you could go anywhere in the world, complete any activity while you were there, and bring any two people (excluding family members, friends, and deities), where would you go, why would you go, whom would you take, what would you do there, and how long would you stay?

Essay #3: Describe a time when you failed. What were you hoping to accomplish, what went wrong, what would you have done differently, and how has this event changed you?

Essay #4: Imagine it is the year 2055 and you have just completed an interview with your local news media because of your recent successes. Show how the media would report your interview by providing a transcript of the interview.

RECOMMENDATIONS AND TRANSCRIPTS

In addition to the above information, two recommendations are required. At least one of these recommendations must be from one of your high school instructors.

The Guidance Department at Sacred Heart High School is able to automatically forward copies of the recommendations from faculty and staff and academic transcripts used for your college applications directly to The Thomas and Elsie Deeley Foundation. For this reason, when you submit an application and receive an email confirming receipt of your application, a copy of this email is also sent to the Sacred Heart Guidance Department. When they receive this confirming email, the Guidance Department automatically forwards copies of your recommendations and transcripts directly to the Deeley Foundation via electronic means.

If you wish to submit recommendations from other sources, please forward them to either the email or postal address above.

We strongly recommend that all students complete the FAFSA (Free Application for Federal Student Aid) Forms and the CSS (College Scholarship Service) Profile Forms. Completion of these forms is required for those wishing to be considered for many of the scholarships from The Thomas and Elsie Deeley Foundation and our partner colleges and universities.

Please note that not every school uses both forms and applicants should check the specific requirements for the colleges/universities they are interested in applying to.
